Certification Renewal Form
75-Days Prior to Expiration:
1. Gather proof of 10-CEU or 10-continuing education workshop hours within the past 2-years
2. Complete the form on the page
3. Wait 30-days for CEU verification and recertification completion
4. Receive 2-year certificate renewal
If your doula certificate has been expired for more than 90-days, recertification is not possible. You will need to retake the doula training course.
